Waiver Agreement: I fully understand that there are risks involved with my participation, and/or my child’s participation. I release and discharge Corporate Sports Inc., DAREtoLOVE Sports Club, the Founder, all executives, staff, volunteers, coaches, officials, coordinators, affiliates, the Dufferin Peel Catholic District School Board, the Peel District School Board, and all facility permit providers from all actions, suits, and demands whatsoever in law or in equity, including by not limited to the risk of injury from participating at any of the said entity’s event(s), activity, activities, and/or programs) and the risk of loss of personal property by theft or otherwise. We cannot permit your participation at this event without your media release consent.
Media Consent: With my participation and/or submission of this Registration; I give Corporate Sports Inc., Corporate Sports Plus, DARE to LOVE Sports Club, PKLballWorld.com, PKLballPlus.com, and their affiliates, the right to reproduce, use, exhibit, display, broadcast, distribute and create derivative works of the photographed or filmed images and likeness of myself and/or my child or children taken during any Corporate Sports Inc. or DARE to LOVE Sports Club event(s), activity, activities, and/or program(s), for promoting, publicizing, marketing and advertising purposes without monetary compensation or otherwise. We cannot permit your participation at this event without your media release consent.
Refund Policy: There are no refunds within 14 days of the start of the program. However, you may find a sub and they can pay you for days missed.
